THE USER / PATIENT acknowledges and certifies that they understand that eligibility for any service, treatment, protocol, or modality of care is subject to professional clinical judgment, availability, safety, contraindications, medical history, and other criteria that the Clinic determines reasonably and in accordance with the law. The presence of information about a service on the website does not mean that such service is appropriate, available, medically indicated, or approved for any particular individual. Any physician–patient relationship may arise only when, as applicable, there has been an adequate clinical evaluation, formal case acceptance, informed consent, eligibility verification, compliance with internal processes, and any additional requirements deemed necessary by the Clinic and the treating professional. THE USER / PATIENT expressly waives any claim that a website visit, preliminary digital inquiry, automated exchange, advertisement, or form submission created a full clinical relationship or an individualized professional duty beyond what is strictly required by law.
5. NO GUARANTEE OF RESULTS
